Bridging the gap: Two examples of innovative practice - Embedding mental health practitioners into local authority homelessness services; and VCSE support into hospitals (Aldgate)

A Talk by Megan Ward and Jon Pritchard

About this Talk

This session will showcase best practice integrated working between a NHS Mental Health Trust and a Local Authority homelessness team to better serve some of their most vulnerable residents. By embedding a mental health clinician in to the Local Authority housing team, the previous challenges and frustrations of silo working have been overcome. The outcomes achieved for, and by, the individuals are phenomenal.

During the workshop, Southern Health NHS Foundation Trust and New Forest District Council will:

  • Share details of the historical challenges and obstacles to joint working.
  • Describe the model, it’s impact on service users, colleagues and the organisations as a whole.
  • Provide an interactive opportunity to explore your own partnership working arrangements, and create time and space to make new connections within the room.

Details will also be shared by Southern Health of their other ground-breaking approaches to addressing the social stressors of their service users, through partnering with VCSE organisations.
